AUDIT READINESS

Physical therapy documentation audit: review risks before the note is signed

An audit after a denial can reveal documentation problems late. A pre-signature review is a different workflow: identify possible gaps while the clinician can still verify the details of that visit.

01 / THE GUIDE

A practical documentation review checklist

Record consistency

Do the intervention descriptions, codes, modifiers, timed minutes and selected units agree in the available documentation?

Clinical context

Does the chart tell a coherent story about functional needs, goals and skilled services, including relevant progress reports?

Plan and certification

Are applicable plan-of-care and certification requirements tracked across the record? A single-note scanner should not claim to validate documents it cannot see.

Clinician review

Is every proposed change accurate, with a final human review before signing or billing?

02 / THE GUIDE

Pre-signature vs retrospective audit

Before signature

The clinician can review a potential issue while details are fresh and correct errors without changing the facts of care.

After claim submission

Billing teams and compliance specialists may have claims, remittances and other evidence available; this is not the same process as reviewing a single note.

Audit defensibility

A tool should explain its finding and cite a rule or rationale so staff can verify it, rather than outputting only a score.

What Verq Check does

Inside WebPT

The extension reviews the available clinical note and surfaces possible documentation risks beside the workflow.

Clear scope

It is not a claim clearinghouse, payer adjudicator, or guarantee of reimbursement; it also cannot verify missing documents that were not supplied for review.

COMMON QUESTIONS

Frequently asked questions

Can AI replace my compliance officer?

No. It can assist review, but human clinical and compliance oversight remains necessary.

Will an audit tool prevent every denial?

No. Denials can arise from eligibility, authorizations, coverage, coding, and payer processes beyond the note itself.
